Eligibility

 

Life Skills' Community Living program offers your loved one support to live independently within the community.  The first step in this process-in order to have the services funded by St. Louis County, St. Charles County or St. Louis City-is to ensure you have a diagnosis that qualifies you for Regional Office Case Management. The next step is for Life Skills to evaluate your needs by utilizing an assessment tool provided by our funders. This assessment will look at your financial ability to live independently, assess your capability to live on your own, and identify the areas where support will need to be provided.  Upon completion of the assessment and approval by the funder, a Life Skills Community Living staff will be assigned to work with you based on your individual support needs.

 

Unfortunately, there is never enough funding to support everyone, creating a waiting list with the Department of Mental Health. Your DMH case manager can walk you through the process, but Life Skills is always willing to assist families with navigating the system.
